2025 Open Enrollment is Here!

Open Enrollment for all benefit-eligible Faculty and Professional Administrative staff begins Monday, October 28 through Friday, November 15. After November 15, no other changes for the 2025 plan year can be made unless you experience a qualifying life event. 

 **Open Enrollment is passive. If no changes are necessary and you do not want to enroll/continue an FSA or HSA, please only ensure your beneficiaries are current in the Maxwell portal and profile information is correct.

What can I change during Open Enrollment?

  • Changes to be made via Maxwell Health
    • Medical, dental, vision, and life insurance enrollment
    • Add/remove dependents
    • Enroll/continue enrollment in a Medical Flexible Spending Account (FSA), Dependent Care Flexible Spending Account (FSA), or Health Savings Account (HSA).
      • FSA 2025 Contribution Limits: Health = $3,300  |  Dependent Care = $5,000 |  Carryover from 2025 to 2026 = $660
      • HSA 2025 Contribution Limits: Individual = $4,300  |  Family = $8,550  |  Over age 55 = Additional $1,000/year
  • Changes submitted directly to kylee.legino@unco.edu
    • Documents required when adding dependents (enrollment completed in Maxwell Health)
      • Spouse = Copy of marriage certificate or civil union certificate + financial/utility document dated within last 60 days with both names - or - redacted 2023 tax return
      • Children = Copy of birth or adoption certificate
    • Copy of current medical insurance card if waiving coverage
      • Waiver of coverage must also be completed in Maxwell Health. All plans can be waived except for Long Term Disability and Basic Term Life which are mandatory for UNC benefit-eligible Faculty and Professional Administrative staff.
    • Section 125 Pre-/Post-Tax document: This allows you to change your medical, dental, and vision premiums from pre-tax to post-tax (or vice-versa). This document must be filled out to make this change versus making the change in Maxwell Health as in previous years.
    • Retirement plan election form if switching ORP vendors. Note: This form does not automatically roll your funds over. You will need to work directly with the vendor you are switching from to begin the rollover process.

  • Three Medical Plans
    • The Blue Advantage HMO/POS is an HMO (Health Maintenace Organization) plan within the state of Colorado and acts as a POS (Point of Service) plan outside of Colorado. The plan contains a $500 individual/$1,000 family deductible for facility services only (excluding Emergency Room and Urgent Care visits). The out-of-pocket maximum is $3,500 individual/$7,000 family. This plan primarily consists of co-pays for in-network providers.

    • The PRIME Blue Priority PPO contains a $600 individual/$1,200 family deductible with a $3,500 individual/$7,000 family out-of-pocket maximum. Once the deductible is met, coinsurance is 15% for covered services.

    • The 2500 High Deductible Health Plan contains a $2,500 individual/$5,000 family deductible with a $3,500 individual/$7,000 family out-of-pocket maximum. Once the deductible is met, coinsurance is 15% for covered services. This is the only medical plan compatible with UNC’s Health Savings Account.

  • Prescription Drug Coverage

    Prescription drug coverage has five tiers:

    • Tier 1: Generic
    • Tier 2: Preferred Brand
    • Tier 3: Non-Preferred Brand
    • Tier 4: Specialty Preferred
    • Tier 5: Specialty Non-Preferred

    To help determine your cost for your prescriptions, you can view your Essential Formulary via BeneCenter or Anthem's website.

  • Dental Plan 
    • To find in-network providers on anthem.com, please search under the “Dental Complete” network.
    • Employees must be enrolled in at least self-only coverage if enrolled in a medical plan.
    • In-network deductible is $0 individual/$0 family.
    • Cleanings for adults will be limited to the industry standard of two per year with exceptions for those who are pregnant or have specific medical conditions.

  • Vision Plan
    • To find in-network providers on anthem.com, please search under the “Blue View Vision” network.
    • Annual vision exams result in a $15 copay but are then covered at 100% for in-network providers every 12 months.

  • Anthem Insurance Cards

    New insurance cards were issued in 2024 due to the changes in deductibles and out-of-pocket maximums. Insurance cards are provided to each member and their dependents. Your insurance card will contain your medical, pharmaceutical, dental, and vision information. You can use your member ID found on your insurance card to create an account on Anthem.com or to download Anthem’s app, Sydney. By having an account, you will be able to easily access your digital insurance card, view claims, keep track of spending, look up providers, and have access to various resources.

  • Wellness Program

    Anthem’s Wellness Reward Program allows members and their legal spouses enrolled in one of UNC’s medical plans to earn wellness incentives of up to $225 a plan year by completing specific wellness activities.   • Infertility Coverage

    In 2023, an addition to your medical plan was diagnosis and treatment of infertility coverage as a result of the passing of Colorado House Bill 22-1008.  Limitations apply. Please review the summaries of benefits & coverages in BeneCenter for additional information.

Enrollment Process

Maxwell Health is our online platform that allows you to quickly shop for benefits, set up profile information, and designate your beneficiaries for your life insurance plans with UNC.  Profile information and beneficiary information may be updated at any time throughout the plan year. However, should you experience a qualifying life event, please contact Kylee Legino at kylee.legino@unco.edu as soon as possible to have your portal opened for changes. Most qualifying event windows are 31 days from the date of the event to make changes. If the window is missed, the next opportunity to update your elections is during Open Enrollment in November.

Enrollment Specifics

  • Open Enrollment

    Outside of a qualifying life event--such as marriage, birth, dependent age off, etc.--Open Enrollment (which typically runs for three weeks in November) allows you to make changes to your plan that you otherwise are unable to make during a plan year, including the following: 

    • Change medical plan type. 
    • Switch medical/dental/vision premiums from pre-tax to post-tax (and vice versa). 
    • Enroll in ancillary coverage through SunLife: Additional Voluntary Term Life & AD&D, Hospital indemnity, Accident, and Critical Illness. 
    • Add/remove dependents. 
    • Enroll/Continue enrollment in a Flexible Spending Account (FSA).
      • The 2024 limit is $3,200 for medical FSA’s and $5,000 for dependent care FSAs. Enrollment is not passive, meaning employees must re-enroll each year to continue this benefit. Please note that medical FSA’s have a “use it or lose it” provision, allowing you to carryover up to $640 of unused money from 2024 into the 2025 plan year. All other unused funds will be forfeited if not claimed by April 15, 2025 for expenses incurred from January 1, 2024 to December 31, 2024. Dependent Care FSA’s do not have a carryover provision and the maximum your household may contribute is $5,000. The 2025 limit is $3,300 for medical FSA’s with a $660 carryover limit of unused 2025 funds into 2026. The dependent care limit is remaining at $5,000 per household in 2025.
    • Health Savings Account
      • If you are planning on enrolling in the High Deductible Health Plan, please be aware that this is compatible with UNC’s Health Savings Account through WEX Health. HSA’s are optional and you do not need to enroll if you are enrolled in the HDHP. UNC does not contribute to HSA’s and the maximum an employee can contribute in 2024 is $4,150 for employee-only contributions and $8,300 for family contributions. The 2025 individual maximum is $4,300 while the family maximium is $8,550. You must be enrolled in a family medical plan to be able to contribute up to the family maximum for your HSA. Unlike the FSA, HSA’s do not have a “use it or lose it” provision and contributions can be changed during the plan year without a qualifying event. 
    • ORP vendor change – This may only be changed during Open Enrollment by completing UNC’s retirement plan election form and returning it to kylee.legino@unco.edu via email or in-person at Carter Hall, room 2002.

  • Retirement Options

    Retirement contributions are mandatory at UNC and are in lieu of Social Security contributions. Employees may choose one of four retirement vendors: PERA (you must have already been in PERA for one year to enroll and are active), TIAA, MetLife, or Corebridge.  PERA is a defined benefit (pension) plan that requires employees to contribute 11% of their pay while UNC contributes 11.61%. The Optional Retirement Plan (ORP) is a defined contribution plan and is a 401(a). Employees enrolled in this plan contribute 8% of their salary to TIAA, MetLife, or Corebridge while UNC contributes 11.5%. If an employee is eligible for both PERA and the ORP, they must make a one-time election between plans. After this election is made, it cannot be reversed.

    Supplemental Retirement Plans

    Employees may open a supplemental plan at any time of the year if they wish to contribute additional funds to retirement. Supplemental plans contain strictly employee contributions as UNC does not contribute to these plans. Contributions can be changed throughout the year by updating your salary reduction agreement and submitting it for processing to Kylee Legino (kylee.legino@unco.edu). All supplemental plans offered through UNC have traditional (pre-tax) and Roth (post-tax) options. The 2024 maximum for 401k, 403b, and most 457 plans is $23,000 and the 2024 catch-up contribution maximum for those 50 and older is $7,500.

  • Life Insurance & Additional Benefits

    During Open Enrollment, you also have the opportunity to elect additional coverage for you and your family through SunLife. To learn more, please review SunLife’s 2025 Benefits Booklet.

    All benefitted faculty and professional administrative staff have a basic life insurance and AD&D policy. Coverage is twice your annual salary to a maximum of $500,000.  Coverage drops to $50,000 at age 67 and to $10,000 at age 70: Monthly premium is $0.14 per $1000 of coverage.

    In addition to your basic term life insurance, all benefitted faculty and professional administrative staff are enrolled in Long Term Disability coverage. This coverage provides up to 66 2/3% of your monthly salary on the 91st day of disability: The monthly premium is 8.4 cents per $100 of monthly salary. 

    Any enrollment/changes in enrollment will be submitted through the Maxwell Health portal.
