Confidentiality Statement
As part of your employment with the University of Northern Colorado, you will at times, be given access to information that is considered Private Health Information. As a UNC employee you have an obligation to:
- Hold all confidential information in trust and strict confidence and agree that it shall be used only for the purpose to fulfill employment obligations; only if you need it to do your job
- Keep all confidential information in your control or possession in a physically secure location
- Not remove any confidential information from UNC unless, and to the extent that, you obtain UNC's written pre-authorization
- Maintain the absolute confidentiality of personal, confidential and proprietary information
- Comply with all privacy laws and regulations
All UNC employees with access to Private Health Information must sign a Confidentiality Statement that will be kept in your UNC employee file. Be aware that a breach of confidentiality or misuse of information will result in disciplinary action up to and including termination of employment.
Note: The Confidentiality Statement should be signed after you have recieved your required annual HIPAA training. Supervisors may contact the HIPAA Office to schedule training for staff or to review self audits.
